Physicians and advanced practice providers (clinicians) dedicate their careers to caring for you, your family and your community. Envision exists to support clinicians throughout their careers so they can focus on providing you care and improving the practice of medicine. If an Envision-affiliated clinician cared for you, you will receive a bill indicating your patient responsibility for their professional services. The billing process is part of the patient care journey, and we strive to provide a compassionate and supportive experience. We are committed to protecting patients from surprise medical bills. Understanding Your Bill

 

You received care from a clinician during your hospital visit.

 

In some circumstances, clinicians are not direct employees of the hospital but rather have a contractual relationship with the hospital to provide medical care at the facility.

 

These clinicians bill separately from the hospital, which results in you receiving a separate bill from your clinician (professional fees versus facility fees, respectively).

 

If you are insured, we already submitted the bill to your insurance, which determined your patient responsibility. Your insurer should have sent an explanation of benefits to you.

 

This bill is for the portion you owe for the professional fees associated with the care you received from an Envision-affiliated clinician.

We are National Partners for the Practice of Medicine with clinicians specializing in anesthesiology, emergency medicine and hospital medicine. We are guided by a shared commitment, to act as trusted partners, to empower clinicians, deliver exceptional patient care and create a healthier future for all.
This bill is for the medical care you received from an Envision-affiliated clinician during your hospital visit or stay, which is most often referred to as “professional fees.” The clinicians who make up the clinician practice are affiliated with Envision and not employed by the hospital. Since the hospital and clinicians are paid separately, you might receive more than one bill – from the clinician for their professional fees and a separate bill from the hospital.
Your insurance company applies the benefits of your plan and then determines your patient responsibility. We worked directly with your insurance company prior to sending a bill. This bill reflects the portion of the cost that is your responsibility to pay directly to the clinician practice that provided your care. If you believe your insurance company made an error, please contact them directly.
While some Envision-affiliated clinicians are not in network with all insurance companies, Envision does not send “surprise medical bills,” also referred to as “balance bills.” If you have insurance coverage but have not yet met your deductible or co-insurance, the amount you owe is determined by your insurance company. Once your insurance company determines what portion of the bill is your responsibility, we will bill you directly for this portion. Your insurance company is required to comply with state and federal law, including the No Surprises Act, in determining the amount of patient responsibility or cost sharing you may owe in out-of-network circumstances.

Envision continuously works with insurance companies so clinicians can participate in their networks. Envision remains committed to protecting patients from surprise medical bills and supports the bipartisan No Surprises Act.
This is not a duplicate bill. Charges for clinicians who care for you during a visit or admission to the hospital may be separate from those of the hospital where you received care. You may receive separate bills for the same visit or admission from the facility and other clinicians who treated you.
During your visit to the hospital or urgent care clinic specified on your bill, you received care from a medical specialist who is a member of an Envision-affiliated practice. You may not have interacted with the clinicians who provided input into your case.

Envision does not balance bill patients. However, when patients receive care from out-of-network clinicians, they may experience higher deductibles or coinsurance due to their insurance company’s plan design. In some circumstances, insurance companies do not provide clear or accurate information regarding your coinsurance and deductible. If you believe your bill is incorrect, or you would like to speak with Envision about a payment plan or support with your bills, please contact us.
Envision has a longstanding policy of not sending “surprise medical bills.” We do everything in our power to protect patients from unexpected costs. Ultimately, your insurance company determines how much it will cover for your medical care and the amount of that care you are responsible for as a patient. For example, your insurance company may require you to pay a high-deductible amount or coinsurance that may not be what you expected. If you believe your insurance company made an error in assigning amounts to you, please contact them directly.

Often, more than one anesthesiology clinician may care for a patient during their treatment. When this is the case, separate invoices are generated for each clinician, but the fee – or amount the patient is responsible for – is split among the clinicians.
Doxo is a company that collects payments from patients and sends those payments to providers like Envision. Doxo typically charges a fee to make those payments. Envision has absolutely no affiliation with doxo. You are not required to use doxo (or any other service) to pay your Envision bill. You can pay your Envision bill directly to Envision without any charge for making that payment.
